According to the Institute of Medicine’s 1999 report, "To Err is Human: Building a Safer Health Care System," as many as 98,000 hospital patients die from preventable causes each year. That figure does not include patients who die in other medical facilities, like nursing homes, or patients who die because of negligence committed in the doctor’s office.
